POSITION SUMMARY:
The certified nursing assistant provides resident care for all of the activities of daily living to the residents of McLean. Takes direction for this care from charge nurses, nursing supervisor and licensed therapy staff. Takes all reasonable steps to ensure the quality of care within the scope of this job description in the provision of this care. Follows McLean Nursing philosophy of Excellence, Leadership, Integration, Learning and Discovery.
JOB FUNCTIONS AND COMPETENCIES: (This may not include all of the duties assigned.)
1. Arrives on unit on time to begin assignment. Engages in proper hand-off to the next shift. Manages work day so as to complete duties within eight hour shift. Accepts re-assignment as needs require. Engages in proper hand-off to the next shift.
2. Responsible to display professional behavior in both communication and behavior.
3. Follows care plan and notes resident’s functional ability; gets report from nurse; makes out a work sheet and reports changes or concerns to charge nurse during the shift.
4. Ensures residents receive support and care during their shift.
5. Assists residents with all of the activities of daily living, noting special instructions on care assignment, i.e. feeding, dressing, toileting, and ambulation, and personal preferences.
6. Takes vital signs as indicated by the plan of care or under the direction of the licensed staff and brings them to the charge nurse in a timely manner.
7. Provides information to charge nurse or MDS coordinator for patient planning conferences and describes resident’s response to plan.
8. Prior to any breaks and after breaks, makes rounds of residents per shift schedule, i.e at the beginning, during shift and when changing shifts.
9. Maintains a safe environment for residents, visitors, and co-workers. If concerns are noted, bring to the charge nurse.
10. Reports changes in resident’s condition promptly to the charge nurse.
11. Follows Standard Precautions, including proper hand-washing to ensure no spread of infection and reports to charge nurse or supervisor when precautions are not being followed.
12. Responsible to wear care unit pagers and is required to respond when signaled, except during meal periods.
14. Records appropriate information on resident flow charts.
15. Takes responsibility for assisting in ordering and monitoring use of supplies as specified in unit activities. Fills portable O2 packs and assist residents in changing from concentrator to portable.
16. Assists peers with resident care and with unit secretarial duties as directed. Accepts feedback from peers, charge nurses and supervision.
17. Performs other activities within the scope of their competency and legal restrictions as directed by the charge nurse, supervisor or Director of Nursing.
18. Attends and completes educational opportunities to satisfy the required 12 in-services and mandatories to maintain certification each fiscal year. Notifies the nurse educator if additional education is needed to perform responsibilities.
19. Attempts to attend at least two meetings annually presented by CEO and/or administrator to stay informed and keep lines of communication open.
Connecticut Nursing Assistant Certification required.
